I've noticed this, too. I always put {CUSTOM_SECTION_NAME} into each "custom section name" on the template. They don't show up in the dropdown list of assignable content areas until I do this. It sounds like you just have to put something in there for them to work.

Ditto to the splash screen comment. It serves no purpose (which is usually the case) and I imagine the click-through rate is not 100%. The template at the bottom determines what the e-mail that is sent to you looks like. Here's an example of what you may want to use for a text template:

name .......... {NAME}
e-mail ......... {EMAIL}
phone ......... {PHONE}
comments ... {COMMENTS}

It's not really possible to edit an external file, although you may be able to create a blank template and enter the xml data onto a page using this template. I've never tried this, but in theory it has about a 75% chance of working.
